The ideal candidate will have strong communication skills and at least three years of nursing experience in Canada. You’ll collaborate with healthcare providers to support individualized care plans and improve patient health outcomes. Key Responsibilities:

- Serve as primary contact during patient care journeys
- Provide virtual case management via phone and digital tools
