Alberta Health Services in Ponoka is seeking an experienced Registered Nurse - Case Manager for the Ponoka Home Care program. You will assess client needs, set care goals, plan, deliver and coordinate services within skilled standards, and collaborate with an interdisciplinary team to ensure holistic, patient-centered care.

This casual/relief role requires on-call availability within 30-45 minutes, CRNA registration eligibility, and a current BCLS-HCP certification.
