Neighborhood Healthcare PACE seeks a Referrals Specialist to coordinate referrals and follow‑up care for our participants. You will receive, process, schedule, and track referral requests for diagnostic testing and specialty care, working onsite in a clinical setting.

This is a full time temporary role lasting about 3 months, with a Monday–Friday, 8am–5pm schedule. You will liaise with providers, manage transportation, and maintain EMR records to ensure timely authorizations and high‑quality
