Our partner is looking for a Senior Nurse Case Manager - Rare Disease based in Canada. You will manage patient cases across an assigned geography while partnering with physicians, hospitals, clinics, insurers, pharmacies, and other healthcare stakeholders. Your work will help coordinate reimbursement, medication access, clinical monitoring, adherence, and resolution of treatment-related challenges.
You will combine clinical expertise with robust communication and case-management skills to support both patients and healthcare professionals. The position also involves contributing to disease-management programs, training, and continuous improvement initiatives. It is well suited to an experienced bilingual nursing professional who is compassionate, organized, and comfortable working independently.
Provide clinical support and education to patients and healthcare providers regarding diseases, therapies, treatment management, and available support services. Deliver inbound and outbound telephone support, including patient follow-up, maintenance, adherence, and other case-management calls. Apply established case-management protocols covering clinical monitoring, data collection, patient follow-up, compliance, therapy interruptions or discontinuation, and adverse event reporting.
Coordinate the documentation and activities required to secure prescription drug coverage through private, public, and federal funding programs. Work with insurers, physician offices, healthcare professionals, pharmacies, and agencies to identify reimbursement solutions, coordinate medication delivery, and resolve access-related issues. Maintain accurate patient and case information, manage databases, and complete ongoing file and data audits.
Communicate clearly with managers, team members, and relevant stakeholders regarding patient cases and case-management activities.
Attend patient meetings, conferences, and specialized therapy training while developing relationships with patient associations, advocacy organizations, and pharmaceutical partners. Collaborate with program colleagues to develop and implement disease-management initiatives, patient materials, compliance parameters, needs assessments, and protocols.
Support the onboarding and development of new Nurse Case Managers through activities such as shadowing and call monitoring. Fluency in both French and English is mandatory. Bachelor’s degree in nursing or a Registered Nurse diploma; At least 5 years of nursing experience.
Experience in healthcare and case management within a medical or hospital practice, insurance environment, or biopharmaceutical setting.
Experience with third-party reimbursement, pharmaceutical sales, managed care, clinical support, or a related field is an asset. Ability to understand complex medical information and communicate it clearly and appropriately to patients and healthcare professionals. Strong patient focus, customer-service orientation, telephone etiquette, and written and verbal communication skills.
Proficiency with Microsoft Outlook, Word, and Excel, along with strong internet research and general computer skills.
