Position Summary The Service Engagement Navigator leads and coordinates practical application of Individualized service planning (ISPs) into community activities including outcome tracking, required consents, schedules, documentation, and follow-up. When specialized input is required, the position works with the Community Engagement Planner, Family Navigator, Complex Supports Navigator, and other professionals and ensures agreed recommendations are incorporated into the Individual Accessing Service (IAS) overall service plan.

Responsibilities

- Promote Meaningful Engagement and Participation for IAS
- Individualized service planning and implementation
- Maintain high-quality documentation, outcome tracking, and service follow-through
- Promote health, safety, and risk prevention
- Supervise assigned workers and build service capacity
- Manage contractors assigned to the position's caseload
- Collaborate with specialized roles, external providers, and service systems

Qualifications/Experience

We are not looking for potential - we are looking for capability.

- Two-year diploma in Human Services, Disability Studies, Community Rehabilitation, Social Work, Psychology, Child and Youth Care, Education, or a related field; a related degree is preferred.
- Equivalent combinations of relevant education and experience may be considered.
- Minimum 3-5 years of relevant experience in disability services, community services, residential supports, case management, service coordination, or a related human services setting.
- Demonstrated experience developing, coordinating, or monitoring individualized support plans, goals, outcomes, and service documentation.
- Demonstrated experience coaching or supervising workers or coordinating teams responsible for individualized services; formal supervisory experience is preferred.
- Experience overseeing contractors, respite services, or other contracted human services is an asset.
- Working knowledge of developmental disability services, person-centred planning, informed consent, health and safety, incident response, and service-quality requirements.
- Knowledge of Alberta PDD services, CET standards, and related disability service systems is an asset.
- Must maintain required Vantage mandatory training and any role-specific training required to perform assigned duties.
- Must have a clear Vulnerable Sector Check.
- Proficient with Microsoft 365 and electronic documentation or data-management systems, or able to learn these systems effectively.
- Eligible to work in Canada

Working Conditions

- Full-time position of 37.5 hours per week.
- Hybrid work may be available following completion of the probationary period and based on organizational and service requirements.




- Flexibility in schedule and working hours is required due to meetings, service needs, on-call responsibilities, and occasional support outside regular office hours.
- Participation in the organizational on-call rotation is required.
- Regular work occurs in office, residential, and community settings according to the needs of assigned IASs and programs.
- Must have access to a reliable vehicle for work purposes and maintain required automobile insurance with a minimum of $2 million liability coverage.
- Must possess a valid Alberta Class 5 driver's licence and provide an acceptable driver's abstract.

What Success Looks Like In this role, you will:

- Turn individualized service plans into clear, practical day-to-day supports that reflect each IAS’s goals, preferences, strengths, and desired outcomes.
- Keep services organized, consistent, and responsive by coordinating schedules, monitoring service delivery, and following concerns and required actions through to completion.
- Maintain high-quality documentation and outcome tracking, using service information and feedback to identify gaps and improve service quality.
- Coach and support workers and contractors to understand expectations, implement plans consistently, and provide accountable, high-quality individualized services.
- Build strong coordination across IASs, families, workers, specialized Vantage roles, and external providers so responsibilities are clear, changes are communicated, and service needs do not fall through the gaps

What This Role Is NOT

- Not an entry-level role
- Not a desk-based or administrative-only role for having regular involvement with IASs, workers, contractors, and services in residential and community settings is required
- Not a role where you simply create service plans and hand them off for you are expected to monitor implementation, outcomes, and follow-through.
- Not suitable for someone who avoids accountability, difficult conversations, or addressing service and performance concerns.
- Not a predictable Monday-to-Friday role with fixed hours for priorities may shift based on service needs, staffing, incidents, and changes in an IAS’s circumstances.
- Not a role for someone who waits for others to identify problems for you will be expected to recognize service gaps, documentation issues, risks, and emerging concerns and take appropriate action.
- Not suitable for someone who requires frequent direction or close day-to-day supervision for the role requires sound judgment, organization, initiative, and knowing when to escalate concerns
