Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care.We are looking for a Social Worker II to join our Supportive Living team to provide care and support to adults and seniors residing in congregate living settings.
Supportive
Living is a non-institutional alternative for those who are no longer able to live in their own home, which focuses on health promotion, well-being, independence and maintaining functional abilities.Working collaboratively as part of a multidisciplinary team, the Social Worker II is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources.
Social
Worker II work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Social Worker is responsible for the provision of social work services to clients and families including psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, and making appropriate referrals to members of the multidisciplinary team. As a Social Worker II,
you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families.
Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work.Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Basic Cardiac Life
Support- Healthcare Provider (BCLS-HCP).The candidate will have proven communication skills to work cooperatively and effectively with the healthcare team members, patients and their families in a fast-paced environment.At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands‑on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.
Preferred Qualifications: Experience in a Continuing Care setting.Knowledge of Connect Care.Palliative Care Experience. A satisfactory criminal record check and/or Vulnerable Sector Search is required prior to your first day of work.
