About The Hollyburn Society The Hollyburn Society is an independent registered charity serving people across Vancouver’s North Shore. Since 2004, we have helped youth, seniors, families, survivors of violence, and people facing housing instability find safety, stability, housing, and support when they are stuck in a tough situation.

The Homelessness Prevention Program (HPP) Case Manager supports homeless and at-risk seniors and families to attain and maintain safe, appropriate housing while promoting long-term stability and well-being. This role focuses on individualized case management, housing stabilization, service planning, community partnerships, and program coordination to ensure clients receive the support they need to achieve successful outcomes.
This is a 40-hour per week, short-term temporary position providing program coverage during a leave of absence.
Qualifications
- BA, BSW or Community College Diploma in a related field with a minimum of two years experience in social services, working with seniors and individuals experiencing homelessness.
- Knowledge of issues and resources specific to seniors and identified at-risk populations
- Working knowledge of mental health, addictions and elder abuse
- Working knowledge of mental health, addictions and homelessness.
- Familiarity with housing issues, emergency housing options and Residential Tenancy Act.
- Hold a valid B.C. Driver’s License and accessed to an insured vehicle.
Desired Certifications & Skills:
- Hold a current First Aid and CPR Certificate.
- Hold a current Non-Violent Crisis Intervention certificate.
- Have excellent oral and written communication skills and computer literacy.

Key reponsibilities:
- Oversees the HPP Program and temporarily, supports with the CHB-HPP tracking and data entry
- Oversees the division, allotment and availability of HPP subsidies
- Create an individual service plan (ISP) based on potential risks, strengths, needs, abilities and preferences of the client goals with measurable goals and objectives and methods to achieve objectives
- Conducts outreach activities to maximize opportunities for contact with seniors/families that are homeless or at risk of becoming homeless
- Implement the ISP; tasks may include housing searches, assist with moves and set-ups, financial planning, application assistance for funding or service, emotional support, life skill instruction or support, advocacy and connecting clients to community services or social networks
- Conducts outreach activities to maximize opportunities for contact with persons who are homeless or at risk of becoming homeless
- Screens potential housing for safety, sanitation and security, where appropriate
- Initiate or manage integrated case management meetings as required
Pay: $30.38 per hour Expected hours: 40.0 per week
