As a Transition Facilitator, you will support Indigenous communities by addressing challenges around transitioning care from hospitals back to their home environments.
This role collaborates closely with health service providers to implement a community-focused approach to health care, integrating both traditional and modern practices. - Ensure compliance with Ontario Quality Standards
- Collaborate with stakeholders for continuous care
- Facilitate client and family engagement in care planning
- Build capacity through training and mentoring
- Document and report on program impacts
Requirements:
- Relevant post-secondary degree in health/social services
- 3-5 years of case management in community settings
- Excellent organizational and communication skills
- Experience with Indigenous culture and practices
- Valid driver’s license and ability to travel
