Lead change as a Follow Up Support Worker at YMCA, focusing on empowering clients through the Housing First model in Edmonton. Provide hybrid case management and community connection. In this full-time role, you will enhance housing stability for individuals who have transitioned into homes via the Housing First program.
With your trauma-informed approach, you will develop personalized action plans and conduct home visits ensuring clients succeed. Your work will make communities resilient and supportive. Key Responsibilities:
- Deliver trauma-informed follow-up support and advice
- Create and manage Individual Action Plans for clients
- Conduct wellness checks to ensure housing stability
- Advocate for access to community resources
- Support clients in using public transit for appointments
Requirements:
- Degree or diploma in Social Work or related area
- At least 2 years' experience in social work contexts
- Valid driving license and access to a vehicle
- Exceptional organizational and communication skills
- Willingness to work flexible schedule as needed
