Position # : 9010044 Expected Start Date: October 12, 2026 Union: HSAS Facility: Wascana Rehabilitation Centre City/Town: Regina Department: Community Health Services Type: Full time temporary Expected Up to Date: September 30, 2027 FTE: 1 Shift Information: Days, Evenings, Weekends, Stats Number of Hours per Rotation: 112 hours per 3 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Social Worker - BSW $35.137 to $42.966 Travel Required: Yes Job Description: As a member of a multidisciplinary Primary Health Care Team, the Social Worker is responsible for the full scope of case management services in an assigned geographical or program area.
The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge. This is done through collaboration with the family, caregivers, and other members of the multi-disciplinary team.
The Social Worker liaises with other services in an endeavor to provide the right service at the right time and in the right place. The position reports directly to a Primary Health Care Manager. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education Bachelor of Social Work (BSW) Licenses Registered Social Worker (RSW)
Valid Class 5 driver's license Competencies Organizational skills Problem solving skills Time management skills Decision making skills Experience Previous experience with clients that have complex medical and non-medical needs Knowledge and Abilities Knowledge specific to populations being served including Determinants of health.
Demonstrated knowledge and ability for competent case management practice Empathy and respect necessary to engage and develop rapport Team player with ability to build and maintain stakeholder relationships. Advocacy, negotiation, crisis management and conflict resolution Build trusting relationships Proficiency with clinical documentation systems Ability to communicate effectively both orally and in writing Openness to teaching and providing educational opportunities.
