As a critical member of the interdisciplinary team, you will provide social work services in an acute care hospital setting.

Your responsibilities will include performing psychosocial assessments and short-term counseling for patients and families facing complex health challenges. You'll coordinate care plans in collaboration with healthcare specialists and address any service delivery gaps swiftly. Key Responsibilities:
Complete comprehensive psychosocial assessments
Provide short-term support and counseling for grief and end-of-life issues




Develop patient care plans alongside healthcare teams
Cover shifts for other Social Workers as needed
Liaise with community health professionals for resource connections Requirements:
Master's degree in Social Work
Registration with Ontario College of Social Workers
Recent hospital social work experience
Robust communication and problem-solving skills
Experience with electronic documentation, preferably Meditech Support patients through psychosocial challenges by joining the dedicated team at Oak Valley Health.
