The position focuses on providing comprehensive support services, advocating for clients, and collaborating with interdisciplinary care teams. Key Responsibilities:
Monitor and evaluate client progress regularly
Provide information on community services available
Co-lead support groups and education sessions
Assist clients in daily living activities
Communicate effectively across disciplines and teams

Requirements:
Certificate from Health Care Worker program
Minimum of two years’ relevant experience
Robust understanding of mental health and addiction issues
Knowledge of community resources in Vancouver
Valid BC Driver's license necessary
