Shared Health seeks a Team Leader for the HUB–FACT–ACT program in Winnipeg to supervise an interdisciplinary team delivering intensive community supports for individuals with severe mental illness. You will guide intake, assessment, and care coordination, ensuring adherence to recovery principles and provincial standards.
The role requires solid leadership, collaboration with multiple partners, and development of policies for FACT and ACT services across Manitoba.
