Empower the Black communities as an Intensive Case Manager with CMHA Peel Dufferin. This on-site role focuses on holistic care and requires experience in mental health and addictions support.

This permanent full time position involves providing individualized case management to clients with responsible mental health and addiction issues. You will facilitate access to primary care and community services, using a stepped-care approach. Additionally, working within a multidisciplinary team, you'll develop care plans and conduct regular assessments.

Key Responsibilities:
Conduct needs assessments and monitor care plans
Provide task-based interventions related to social determinants of health
Connect clients to crisis services when needed




Collaborate with the multidisciplinary team for care planning
Facilitate client attachment to primary care providers

Requirements:
Post-secondary degree in relevant discipline
Minimum 2 years of experience in mental health
Expertise in community resources and support
Solid cultural sensitivity working with BAC communities
