North Lambton Community Health Centre in Forest, Ontario, is seeking a Fulltime Case Manager for a 1-year contract within the Harm Reduction and Hepatitis C Care Team. The role involves client-centred case management, outreach, advocacy and helping clients navigate testing, treatment and follow-up.

Primary site is Forest with outreach locations; working 37.5 hours per week Monday to Friday. The incumbent will provide education and psychosocial support, and connect clients to health and community
