Position Summary

The Family Connections Navigator helps Individuals Accessing Service (IAS), families, and guardians navigate internal Vantage and external disability, health, social, financial, housing, transportation, employment, recreation, and community service systems. The position supports referrals and applications, identifies appropriate resources, and coordinates warm connections to external providers when additional services or expertise are required. The role strengthens family capacity and confidence while respecting the Individuals Accessing Services (IAS’s) voice, choice, consent, privacy, and right to direct their own life.

Responsibilities

- Build trusting and collaborative relationships with Individuals Accessing Services (IAS), families, guardians, and natural supports
- Provide system navigation, information, referrals, and resource connections
- Support intake, onboarding, transitions, and family engagement
- Coordinate services and planning for IASs assigned to the position's caseload
- Supervise and support workers assigned to the position's caseload
- Support documentation, quality, safety, and organizational improvement

Qualifications/Experience

We are not looking for potential - we are looking for capability.

- Two-year diploma in Human Services, Disability Studies, Community Rehabilitation, Social Work, Family Studies, Psychology, Child and Youth Care, Education, or a related field; a related degree is preferred.
- Equivalent combinations of relevant education and experience may be considered.
- Minimum 3-5 years of relevant experience in disability services, family support, case management, system navigation, community services, or a related human services setting.
- Demonstrated experience working collaboratively with individuals with developmental disabilities, families, guardians, and multidisciplinary or cross-agency service teams.
- Demonstrated knowledge of community resources and experience supporting referrals, applications, service navigation, or coordination across multiple systems.
- Knowledge of Alberta PDD, AISH, Alberta Supports, OPGT, health and mental health services, housing, transportation, and other disability-related service systems is strongly preferred.
- Demonstrated leadership, coaching, mentoring, or team-development experience; formal supervisory experience is preferred.
- Experience managing contractors, respite services, or family-based service arrangements is an asset.
- Ability to manage sensitive information, conflict, competing perspectives, and emotionally difficult conversations while maintaining professionalism and confidentiality.




- Must maintain required Vantage mandatory training and any role-specific training required to perform assigned duties.
- Must have a clear Vulnerable Sector Check.
- Proficient with Microsoft 365 and electronic documentation or data-management systems, or able to learn these systems effectively.
- Eligible to work in Canada

Working Conditions

- Full-time position of 37.5 hours per week.
- Working different shifts including days, evenings and weekends according to program or individual needs due to family meetings, service needs, on-call responsibilities, and occasional support outside regular office hours is required.
- Participation in the organizational on-call rotation is required.
- Must have access to a reliable vehicle for work purposes and maintain required automobile insurance with a minimum of $2 million liability coverage.
- Must possess a valid Alberta Class 5 driver's licence and provide an acceptable driver's abstract.

What Success Looks Like

In this role, you will:

- Build strong, trusting relationships with IASs, families, guardians, and support networks while keeping the IAS’s voice, choice, and goals at the centre.
- Help IASs and families confidently navigate disability, health, social, financial, housing, and community service systems and connect with the right resources.
- Coordinate services, referrals, transitions, and follow-up so responsibilities are clear, communication is consistent, and important needs do not fall through the gaps.
- Support and coach workers to provide consistent, individualized, high-quality services while maintaining clear expectations and accountability.
- Identify concerns, service gaps, and emerging risks early and work collaboratively with families, workers, internal teams, and external providers to find practical solutions and improve service quality.

What This Role Is NOT

- Not an entry-level role
- Not a role for someone without hands-on experience in disability services, family support, case management, or system navigation.
- Not a desk-based or administrative-only role for you will work directly with IASs, families, workers, and community partners.
- Not a role where you simply provide families with a list of resources since you are expected to help navigate systems, coordinate connections, and follow through on next steps.
- Not suitable for someone who avoids difficult conversations, competing perspectives, conflict, or accountability.
- Not a predictable Monday-to-Friday role with fixed hours for the role requires flexibility, on-call participation, and occasional evening or weekend work are required.
- Not a role for someone who waits for problems to be brought to them for you will be expected to identify service gaps, concerns,



and emerging risks and take appropriate action within your scope.
