Qualifications – Ontario Healthcare Facility
- Current registration as a Nurse Practitioner with the College of Nurses of Ontario (CNO)
- Minimum 2 years of relevant experience
- BLS / CPR certification
- Long-Term Care experience required
- Full-time availability
- Monday–Friday schedule
- Start Date: ASAP
- End Date: Permanent

Responsibilities:
Nurse Practitioners are required to adhere to the scope of practice as defined by the College of Nurses of Ontario. Responsibilities may include but are not limited to:
- Provide comprehensive nursing and primary care to residents in a Long-Term Care setting
- Conduct assessments and monitor residents' health status
- Develop and implement individualized treatment and care plans
- Assess and manage acute and chronic health conditions within scope of practice
- Order and interpret diagnostic tests and laboratory results as required
- Prescribe medications and treatments within scope of practice
- Collaborate with physicians, nursing staff, residents, families, and other members of the interdisciplinary care team
- Provide education and support to residents, families, and caregivers
- Maintain accurate and timely clinical documentation
- Follow applicable healthcare regulations, infection prevention practices, and patient safety standards
Note: Duties may vary slightly based on facility and shift, with specific guidance provided during site orientation.
Experience:
- Minimum 2 years of relevant Canadian nursing or Nurse Practitioner experience
- Long-Term Care experience required
- Experience managing complex and chronic conditions is an asset
- Ability to work independently and collaboratively within an interdisciplinary team
- Strong clinical judgment and adaptability
Skills:
- Excellent communication and interpersonal skills
- Strong clinical assessment and decision-making abilities
- Ability to prioritize tasks and manage time effectively
- Strong documentation and clinical record-keeping skills
- Compassion and empathy when supporting residents and families
- Ability to manage a full-time caseload in a Long-Term Care environment
