Position Summary: The Caseworker is responsible for providing comprehensive case management, care coordination, outreach, and navigation services to youth (ages 12-18) who have been diverted from the Justice system and adults who are experiencing homelessness. The Caseworker serves as a trusted partner, helping participants identify strengths, overcome barriers, and access coordinated services that promote health, education, employment, housing stability, behavioral health, and overall well-being. The Caseworker will build collaborative relationships with community providers, educational institutions, workforce partners, and social service agencies to ensure participants receive timely, coordinated, and person-centered care.
This position utilizes trauma-informed, culturally responsive, and youth-driven practices to empower clients toward self-sufficiency and long-term success.
Essential Functions
- Complete participant intake, screening, strengths-based assessments, and eligibility determination for ICS services.
- Develop individualized service and goal plans in collaboration with participants, incorporating education, employment, behavioral health, housing, transportation, and other supportive service needs.
- Provide ongoing care coordination for youth through regular meetings, follow-up contacts, home or community visits (as appropriate), and progress monitoring.
- Serve as the primary point of contact for participants, providing guidance, advocacy, encouragement, and system navigation throughout their participation in ICS Programs.
- Coordinate referrals and facilitate access to internal and external resources, ensuring warm handoffs and continuity of care.
- Monitor participant progress toward established goals and modify service plans as needed.
- Assist participants with crisis intervention, problem-solving, and barrier reduction while maintaining appropriate qualified boundaries.
- Maintain accurate, timely, and confidential participant records, case notes, assessments, referrals, and outcomes in accordance with organizational policies and funding requirements.
- Participate in multidisciplinary team meetings, case conferences, trainings, and community collaborative meetings.
- Perform other duties as assigned.
Education
- Bachelor's degree or Diploma in Social Work, Human Services, Psychology, Counseling, Public Health, Criminal Justice, Education, or a closely related field.
Experience
- One - two years of experience in case management, care coordination, youth services, behavioral health, workforce development, or community-based human services preferred.
- Experience working with underserved populations, or individuals experiencing barriers to education, employment, housing, or healthcare preferred
