Overview

The Client Care Worker (CCW) provides personal care and delegated health interventions to clients living in specialized care units within shelters and supportive housing environments. Working within Carefor policies, procedures, standards, and quality & risk management frameworks, the CCW collaborates with all members of the health care team to support individuals with complex physical, mental health, and substance-use-related needs. This role supports programs within the Ottawa Inner City Health (OICH) network, including shelter-based units such as Men’s Special Care, TED (Targeted Engagement and Diversion), and Hospice, as well as supportive living programs such as Merivale Housing, Carruthers Housing, Oaks, Booth House, and Richcraft Residence. Care is delivered within a Harm Reduction Model, which may include managed alcohol programs and safer supply opioid distribution.

The CCW works under the delegation of a regulated health care professional and plays a key role in promoting client dignity, safety, and quality of life.

Responsibilities

- Provide client-centred support and assistance with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, mobility, and safe transfers (including lifts and mobility aids).

- Provide environmental support such as light housekeeping, laundry, and maintaining a clean and safe living environment.





- Administer and document medications as per the Medication Administration Record (MAR); medications may include naloxone (in overdose emergencies) and insulin (for diabetes protocol).

- Monitor and report changes in client condition; take vital signs as per care plan.

- Provide social and emotional support to reduce isolation and promote well-being.

- Complete electronic charting, MAR entries, and communicate effectively with the care team.

- Follow PPE protocols, infection prevention practices, and health and safety regulations.

- Provide basic ostomy and catheter care; assist with end-of-life care where applicable.

- Participate in multidisciplinary care planning and contribute to service plan development.

- Support harm reduction strategies to enhance client safety and reduce risk.

- Observe and report any changes in client condition, behaviour, or environment to the supervisor.

Qualifications

- Personal Support Worker Certificate, Developmental Service Worker Certificate, Internationally Educated Nurse, or 2nd-year (or beyond) nursing student (essential).

- Ability to work independently and collaboratively in a team setting (essential).

- Clear vulnerable sector police check (essential).

- Experience supporting individuals facing housing insecurity, homelessness, or other social determinants of health (asset).

- Experience working with individuals experiencing substance use, mental health challenges, and complex medical conditions (asset).

- Knowledge of the Harm Reduction Model of Care (asset).

- Physical ability to perform lifting, repositioning, and transferring tasks (asset).





- Ability to work in a fast-paced environment with multiple demands.

- Strong interpersonal and communication skills (asset).

- Experience in home care, retirement homes, long-term care, or community health settings (asset).

- Experience administering or assisting with medication (asset).

- Non-violent Crisis Prevention and Intervention training (asset).

- Gentle Persuasive Approach (GPA) (asset).

- Suicide ASIST training (asset).

- Palliative Care training (LEAP or similar) (asset).

- Valid driver’s licence and access to a reliable vehicle (asset).

- Current CPR and First Aid certification (asset).

- Mental Health First Aid (asset).

- Registered PSW with HSCPOA (asset).

- Bilingualism (English/French) (asset).
