You will offer outreach services primarily to individuals experiencing homelessness, providing episodic primary care and mental health support. This role requires connecting clients with essential internal and external resources while advocating for their needs in collaboration with community agencies. Key Responsibilities:

- Screen clients for eligibility for various health services
- Facilitate links to community and government resources




- Provide health outreach services in shelters and encampments
- Advocate for clients facing complex health challenges
- Collaborate with interdisciplinary teams for optimal outcomes Requirements:
- Grade 12 and Welfare Aid/Social Services Certificate
- 1-2 years working with marginalized populations
