Organization Description:
Emery-Keelesdale Nurse Practitioner-Led Clinic (EKNPLC) offers a current model of primary health care in which Nurse Practitioners (NPs) coordinate client care within a collaborative and vibrant interdisciplinary team environment. As part of our commitment to a philosophy of Primary Health Care we emphasize connecting and coordinating services within the health care system to those from other sectors to address the broader social determinants of health. The clinic operates from both a main site and a satellite site to offer services to individuals living in the North York West community.
EKNPLC is seeking a Health Promoter to join our growing team which services clients in two locations. If you are enthusiastic about improving community health by providing resources and information as well as empowering patients/clients through knowledge then this job is for you. We seek a dynamic individual who will directly design and execute campaigns to advance the ongoing care of our key cohorts of seniors and youth.
Responsibilities/Description:
- Work with community partners and groups to promote involvement and develop supportive environments
- Conduct community needs assessment
- Community Mobilization
- Directly engage and impact the social determiners of healthy communities
- Increase public awareness especially for program priorities of the centre and related care issues
- Develop relevant and accessible health promotion materials
- Educate through conducting promotion events in tandem with interdisciplinary team efforts
- Program planning and evaluation
- Advocacy; engage and encourage healthy public policy
Requirements/Qualifications:
Bachelor’s Degree in Health promotion, Health Sciences, Public Health or related discipline with emphasis on social marketing or health promotion and behavioural psychology and public health
Excellent facilitation, written communication, media design and teaching skills and cross-cultural communication skills
Ability to work in fast-paced and priority changing environments
Ability to work with wide range of interest groups and community partners
Proficient in Public Health Policy
1-3 years relevant experience in community health or public health setting
Knowledge of Anti-Oppressive practices and harm reduction
Proficient in Population Health Assessment
Proficient in Office Suites and social media
Fluent in English, second language an asset
Other Details: Must Have Valid Ontario License and Vulnerable Sector Police Check

Job Type: Part-time
Pay: $76,000.00-$77,673.00 per year
