Horizon Health Network is seeking a Human Service Counselor to assess, support and assist clients in community or hospital settings through therapeutic and rehabilitative programs.

The role includes developing and implementing client support plans, observing changes, and coordinating with a multidisciplinary team to modify services, provide crisis intervention, teach life skills, and help clients access community resources.
