Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across diverse settings, including nursing homes and in-home care environments. Your expertise will help ensure patients receive safe, effective, and personalized care while supporting their physical and emotional well-being.

Responsibilities
- Deliver comprehensive nursing care by monitoring patient health status, administering medications, and assisting with treatments in accordance with physician orders.
- Conduct patient assessments, document medical histories, and update clients' records.
- Assist with laboratory procedures such as phlebotomy and specimen collection to support diagnostic testing.
- Provide specialized care for patients with post surgery care, dementia, hospice needs, or requiring pain management strategies.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans for patients across various settings including acute care, home health, and assisted living.
- Educate patients and their families on medication administration,
disease management, and health maintenance practices.
- Maintain compliance with healthcare regulations and safety standards while ensuring a compassionate approach to patient care.
Skills
- Proven experience in nursing home, hospital, or acute care environments with a strong understanding of medical terminology and patient care protocols.
- Skilled in medication administration, laboratory procedures such as phlebotomy, and case management techniques.
- Familiarity with electronic health record systems and data management.
- Experience working with diverse populations including pediatrics, seniors, dementia patients, and hospice clients.
- Knowledge of pain management strategies and hospice or palliative care practices.
- Ability to communicate effectively with patients, families, and healthcare teams while demonstrating empathy and professionalism.
- Strong organizational skills to handle multiple responsibilities efficiently in fast-paced settings.

Pay: $26.26-$44.49 per hour
Expected hours: 20.0 – 40.0 per week
