Job Overview
PRESS is seeking 2 compassionate and dedicated Nurse Case Managers to join a small care team in North Vancouver. The candidate needs to be a Registered Nurse (RN) or an Unregistered Nurse (i.e. educated as an RN).
Working within a client-centered model of care, as a Nurse Case Manager, you will provide medical assessments, medical monitoring/tracking, judgements, interventions, and health care planning for a young adult who has a multi-domain complex health profile, and who lives in a personal apartment condo. You will also provide medical leadership, including training, direction, and direct and indirect supervision for the other non-nurse team members who provide the young adult's care on a 1 :1 client to care provider ratio, 24 hours per day, 7 days per week.
To ensure work-life balance, you will share the role and work collaboratively with a 2nd Nurse Case Manager, and you will be further supported by young adult's legal representative/parent, who is also a nurse.
In this role, your primary focus will be on improving the health, the overall functioning, and the quality of life of the young adult. This position requires a blend of past complex care experience, the ability to learn client-specific health condition knowledge and skills, and the ability to work in regular collaboration with the young adult, their family, the rest of the care team, the physicians, the other health care providers, and the other community partners.
The ideal candidate has a strong background in caring for individuals with multiple medical conditions and associated medical-related challenges resulting from neurological illness with associated cognitive impairment, emotional and behavioural challenges, endocrine conditions, and other related resultant medical conditions.

Key Duties
- Assist with the development and updates of the written health care plan, the personal care plan, the emotional and behavioural support plan, and associated protocols, tools, and resources, tailored to effectively and safely meet the young adult's individual needs.
- Participate in the development of workplace policies, procedures, and best practice standards to support effective and secure medical care, personal care and emotional and behavioural support.
- Provide the initial orientation education and training and ongoing education, training, and development of the other care team members. Educate them on the young adult's medical conditions, the risks to the young adult, the signs and symptoms to monitor, and the actions to take when needed. Educate and train care team members on specific delegateable tasks, as outlined in the health care plan, the personal care plan, the emotional and behavioural care plan and the associated protocols. Maintain traning records.
- Provide in-person nurse medical assessment and track the young adult's regular and changeable signs and symptoms to support the efficient identification of neurological, endocrine and/or other condition deteriorations.
- Directly implement appropriate medical interventions, and/or direct and supervise delegated medical tasks by the care team member(s). Support referrals to physicians, urgent care, and the emergency department when indicated.
- Support the psychosocial aspects of the young adult's care by assessing and correctly interpreting their emotional and behavioural presentations, and by directly applying specific approaches, responses and strategies to support their emotional and behavioural stability and participation in activities of daily living. Support the other care team members to do the same.
- Support the care team members to maintain clear, timely, and accurate client records, including documentation of all signs and symptoms, medication administration, interventions, and incidents.
- Regularly collaborate with the care team members, the young adult's family, physicians,
and the multidisciplinary healthcare team members to support the provision of effective and safe care.
- Support the young adult's medical care coordination in non-home environments (i.e. during medical appointments, urgent care visits, emergency department visits, and any hospital stays).
Qualifications
- Graduation from an accredited Registered Nurse (RN) education and training program is required.
- Completion of a Bachelor's of Science in Nursing degree or equivalent degree is preferred.
- Current or past registration with a nursing professional college within British Columbia (BCCNM), another province, or from another international jurisdiction is required.
- A minimum of two (2) years' work experience as a nurse is required.
- A minimum of two (2) years' experience working with clients with complex medical profiles is required.
- A minimum of two (2) years' nurse case management experience is preferred.
- Experience and proficiency in developing individualized health care plans, protocols, tools and reference materials is required.
- Proficiency with MS Office (Outlook, SharePoint, Word, Excel, Teams) and electronic documentation systems is required.
- A minimum of two (2) years' experience in a managerial, supervisory and administrative role is preferred.
- Effective verbal and written communication skills are required.
- Experience in educating, training, and delegating medical care to non-nurse care providers and supervising them is required.
- Solid clinical assessment skills, excellent organizational abilities, and attention to detail are required.
- Strong critical thinking, decision-making skills, and problem-solving skills are required.
- Ability to work well independently and within a small team is required.
Certifications
- Standard First Aid and CPR C certification is required (Can be obtained after hiring)
- MANDT System and/or Non-Violent Crisis Intervention (NVCI) certification is required. (Can be obtained after hiring)
- Criminal record check (CRC) by the Ministry of Public Safety and Solicitor General to allow work with vulnerable adults is required. (This will need to be obtained specifically for PRESS after the job offer with an acceptable record being a condition of employment.)
- A driver's license and an acceptable driver's abstract (obtained from ICBC) are required, along with regular access to your own car.
Additional Assets:
- Experience working with individuals with brain injury and associated neurological disorders and endocrine disorders (i.e. dysautonomia, hydrocephalus with shunt treatment, adrenal insufficiency and other hormone deficiencies).
- Experience working in hospital acute care (i.e. neurosurgery unit, neurology unit, emergency department, intensive care unit.), in community health services, in community group homes and/or assisted living facilities.
- Experience working within trauma-informed and emotionally responsive care frameworks.
- Experience working with individuals who have the psychosocial impacts of brain injury, neurological impairment, cognitive impairment, and intellectual disability, and that require adjustments to client care and support.
- Ability to work effectively both independently and as part of a team care in a dynamic environment.
- Commitment to adopt management approaches that support a collaborative, strengths-based, minimal-hierarchical team, and to foster a positive, respectful, fair and supportive work culture.
- Strong interpersonal skills with a focus on empathy and compassion, the ability to build rapport, and the ability to flex in approach and responses to the young adult or care team members for better outcomes.

- Initial client-specific education and training will be provided.
- Ongoing support will be provided.

Salary/Wages:
- This management level position is paid by annual salary (as opposed to by hourly pay).
- The salary is based on this being a shared role by 2 RN's (thus part-time)
- The starting salary compensation is $70,000 per year, paid on a bi-weekly basis.
- The salary compensation may be negotiable depending on the candidate's qualifications and experience.
- Mileage compensation for work-related travel will be provided as per policy.
Schedule:
- The schedule is shared with a 2nd Nurse Case Manager with each nurse normally responsible to cover 1/2 of the week's workday hours plus after hour on-call coverage.
- There is a requirement to cover additional days when 2nd RN Case Manager is unavailable (e.g. during vacation and/or any illness periods).
- On scheduled days, in-person attendance at the client's apartment for medical monitoring and team leadership is required, for the number of work hours required to meet this need.
- As the care team's provision of care services becomes effective and safe, the ability to work more hours from home can be considered.
