Occupational Therapist – GAIN Program – Temporary Full-Time

Primary Duties

· Triage and prioritize incoming referrals.

· Conduct comprehensive geriatric assessments in collaboration with the interprofessional GAIN team.

· Assess cognition, functional abilities, frailty, adaptive behaviours, and living environments to support care planning.

· Develop individualized care plans and recommendations to optimize patient outcomes.

· Participate in case reviews, follow-up visits, care conferences, and interprofessional team meetings.

· Provide education, consultation, advocacy, and support to clients, families, caregivers, community agencies, and health care providers.

· Promote client safety, independence, and quality of life through evidence-based interventions and strategies.

· Maintain accurate clinical documentation and contribute to quality improvement initiatives.

Education & Experience

· Master of Science in Occupational Therapy (MScOT) or equivalent required.

· Current registration with the College of Occupational Therapists of Ontario required.

· Current Basic Cardiac Life Support (BCLS) certification required.

· Valid Ontario Driver's Licence and vehicle insurance with $2 million liability coverage required.

· Valid Police Check required upon hiring.

· Minimum three (3) year recent experience working with a geriatric population in a community and/or clinic setting required.

- Preference will be given to applicants with the following qualifications:

o Ontario Assistive Devices Program (ADP) Authorizer, an asset. Skills & Abilities

· Ability to complete accurate and comprehensive geriatric assessments.

· Demonstrated knowledge of occupational therapy best practices.

· Experience supporting individuals living with dementia, delirium, neurological disorders, and other complex health conditions.

· Ability to apply evidence-based frameworks such as PIECES and other geriatric care approaches.

· Demonstrated commitment to client-centred care planning and service delivery.





· Solid knowledge of aging-related medical and biological issues, including medications, risk factors, and delirium.

· Excellent critical thinking, problem-solving, and troubleshooting skills.

· Strong organizational skills with the ability to prioritize competing demands.

· Excellent interpersonal skills and ability to work collaboratively with patients, families, physicians, staff, and community partners.

· Excellent verbal, written, documentation, and communication skills.

· Ability to work independently and effectively as part of an interdisciplinary team.

· Proficiency with basic computer applications, including EPIC.

· Demonstrated commitment to ongoing professional development and continuing education.

· Demonstrated good attendance and reliability.

Compensation Disclosure: $51.15 to $63.74
