Transitional Care Unit

Full Time

Pay Range: $125,000- $149,000 Annually

Toronto Grace Health Centre (TGHC) provides specialized care to individuals requiring Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional Care, and Palliative Care services. Our programs support patients as they navigate the health care system, recover from illness or injury, and transition to their homes, communities, or Long-Term Care (LTC) settings.

TGHC operates across two locations, Downtown and Scarborough. We are committed to working collaboratively with patients, families, caregivers, and community partners to support safe, effective transitions and optimize quality of life throughout the continuum of care.

The Patient Care Manager (PCM) is an integral member of the interprofessional team and provides leadership and oversight for the clinical operations of the Harbour Light Transitional Care Unit. The PCM is accountable for the 24-hour delivery of safe, high-quality, person-centred care consistent with the mission, vision, values, policies, and standards of Toronto Grace Health Centre.

Position Summary

Reporting to the Director of Clinical Operations and Flow, Toronto Grace Health Centre is seeking a Patient Care Manager (PCM) for the Transitional Care Unit.

The ideal candidate will bring strong leadership experience in aging, complex continuing care, rehabilitation, transitional care, long-term care, or another setting serving older adults with complex health and social needs. Experience in mental health care is considered an asset, particularly where it supports the management of patients with co-existing mental health, behavioural, cognitive, or psychosocial needs.

The PCM applies sound management principles, evidence-informed practices, and a team-oriented leadership approach to optimize patient outcomes, patient flow, quality of care, and resource utilization. The role is responsible for effectively coordinating human, financial, and material resources while fostering a positive, respectful, and supportive practice environment.

As a leader, coach, and mentor, the PCM supports nurses, allied health professionals, and administrative staff in delivering high-quality, person-centred care.

Main Responsibilities:

- Take initiative and responsibility for decisions as an individual and exhibit commitment to personal and organizational goals.
- Participate in the ongoing development of competencies, abilities, technology and the resources to achieve high standards of efficiency and effectiveness.
- Directly supervise employees, responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; evaluating performance; counsel employees when necessary regarding performance concerns; as well as addressing complaints and resolving problems.
- Promote patient focused care and ensures that care and service are developed with the goals of the patient/family in mind.
- Integrate the work efforts and goals of all team members.
- Adhere to the Code of Conduct. Treat employees, patients and families with respect.
- Promote a safe and efficient working environment by adhering to organizational policies, guidelines, infection control and standard precautions.
- Assign, supervise and guide staff in providing high quality care and services including accurate documentation and participation in patient case conference meetings based on clinical competence.
- Maintain confidentiality of all organizational and patient related information. Protect patient rights.
- Maintain proper attendance and punctuality to ensure that the unit is operated in an efficient and cost effective manner.
- Perform on-going review of clinical records of new, active and discharged patients.

Qualifications and Education:

- Master’s degree in Health Care / Nursing preferred.
- Current Certificate of Competence issued by the College of Nurses’ of Ontario in good standing
- Minimum of 3-5 years working in a hospital environment; minimum 2 years of experience leading an interprofessional health care team
- Experience in diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting an asset
- Experience with community-based programs and services preferred
- Self-directed; ability to handle a wide variety of administrative tasks
- Well-organized with an ability to multi-task, prioritize and meet deadlines
- Superior interpersonal skills and high degree of emotional intelligence




- Proven change management skills combined with the ability to work with others
- Proven ability to promote an academic practice environment within the clinical setting
- High degree of professionalism, integrity, maturity, discretion, initiative and judgment
- Self-motivated, team player, and ability to thrive in a fast paced environment.
- Excellent oral and written communication skills
- Demonstrated proficiency in Microsoft Office, advanced knowledge of Word, PowerPoint, Outlook, Excel)
- High level of integrity, leads by example and exemplifies our core Organizational Values
- BCLS Certification
- Excellent attendance record
- Must have physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Occupational Health Services
- Demonstrate membership within a professional association and/or professional interest groups related to practice preferred.
- Client service oriented, with the ability to effectively work within a framework that supports diversity, inclusiveness, and equity.
- Demonstrate respect and support for diversity within and among our patients, families, and teams.
- Must have received at least two doses of a COVID-19 vaccine approved by Health Canada prior to start date and submit evidence of such. Must maintain Covid-19 Vaccination schedules as approved by Health Canada. Medical exemptions or any other kind of requested exemption based upon the Hospital’s obligations pursuant to the Ontario Human Rights Code will be considered on a case-by-case basis.
