Vital Path Care Inc. is an Ontario-based private home and community care provider dedicated to delivering compassionate, reliable, and personalized care to individuals and families across the Greater Toronto Area and surrounding communities.

We provide a range of services, including personal care, dementia support, respite and overnight care, companionship, community assistance, and support for individuals recovering from motor vehicle accidents (MVA). We also support individuals with developmental, neurological, and complex care needs.

Our services extend to seniors, adults, children, and individuals requiring additional support to maintain their independence, dignity, and quality of life.

We work collaboratively with families, legal professionals, rehabilitation providers, and other healthcare professionals to coordinate appropriate care based on individual needs.

As our organization continues to grow, we are expanding our professional caregiver network to support new client referrals and upcoming care assignments.

Who We’re Looking For

We are seeking compassionate, dependable, and experienced healthcare professionals to join our growing care provider network.

Successful candidates will be considered for upcoming home care, community care, respite, rehabilitation support, and other client assignments based on their qualifications, experience, and availability.

- Developmental Service Workers (DSWs)

Key Responsibilities
- Provide compassionate, client-centred care in accordance with individualized care plans.
- Assist clients with activities of daily living, personal hygiene, mobility, and general well-being.
- Support seniors and individuals living with dementia, developmental disabilities, neurological conditions, or recovering from injuries.
- Provide companionship, respite care, and community support as required.
- Maintain accurate care documentation and communicate relevant updates to the care coordination team.
- Promote client safety, dignity, independence, and comfort.
- Collaborate professionally with families and other members of the client’s care team.

Requirements
- Relevant certification or diploma in your field.
- Current CNO registration in valuable standing for RPN and RN applicants.
- Valid CPR and First Aid certification.
- Current Vulnerable Sector Check.
- TB screening and immunization records, where required.
- Previous home care, community care, rehabilitation, or related healthcare experience preferred.
- Strong communication, reliability,



and professionalism.
- Ability to travel to client locations throughout the GTA and surrounding communities.
- Access to reliable transportation is an asset.
