Position Overview
Reporting to the Manager, Organizational Quality and Safety, the Patient Safety and Quality Leader is responsible for providing guidance, support and subject matter expertise in patient safety culture by reducing and preventing events that contribute tounintended adverse patient outcomes. This individual facilitates, mentors and steers hospital teams to build on and improve practices focused on the delivery of high quality, highly reliable patient care that is aligned with the organization’s vision andstrategic goals. The individual will collaborate with multiple teams across the organization to improve the health experience of our patients by contributing to the engineering of safe systems, ensuring that patients receive the safest care, and reducingorganizational risk.
The ideal candidate will support the identification and prioritization of quality improvement opportunities to develop controls and evaluation plans to assure sustained results.
Primary Responsibilities
- Develops and sustains best practices in patient safety and quality by providing guidance,
education and subject matter expertise in the planning and management of quality and patient safety initiatives across the organization.
- Fosters a culture of quality and patient safety across the organization
- Designs and facilitates education in support of patient safety standards and ensures transfer
ofknowledge through follow-up, evaluation and sustainability planning.
- Facilitates the implementationof quality improvement activities and acts as a resource to clinical
program leaders in the development and maintenance of quality improvement processes.
- Leads unit based quality improvement teams in identifying and developing controls and solutionsto
enhance care from a patient safety perspective.
- Leads/participates in process review, patient safety incident reviews and FMEAs related to
quality/safety improvement within clinical programs.
- Develops projects, process improvements and actionplans in collaboration with unit level
leadership to address gaps identified through patient safety events, incident trend analysis and partnershipwith various stakeholders.
- Monitors and ensures compliance throughout the organization, including butnot limited to
specific initiatives related to quality and patient safety.
- Advocates and encourages a just culture of non-punitive error reporting throughout the
organization.
- Collaborates with key stakeholders in the development of policies,procedures and application of best
practices affecting patient and staff safety.
- Seeks to protect and enhance the Hospital's ability to achieve strategic priorities.
- Supports the development and maintenance of integrated risk management processacross the
organization and patient safety related legal claims.
Best Practices
- Develops and sustains best practices in Quality and Patient Safety
- Proactively identifies risks within the organization and works with teams in risk mitigation.
- Oversees the management of liability claims and lawsuits against the Hospital as well as ongoingsupport of staff and managers involved.
- Ensures staff and patient safety is paramount and that systems, policies andp rocedures support thehospital's obligations under the Internal Responsibility System.
- Oversees and directs all activities used to identify, evaluate and take corrective action against risksthat may result in injury to patients, visitors and employees.
- Oversees needs assessments to identify organization-wide Quality/Patient Safety training
requirements for managers and staff and facilitates the design, implementation and evaluation of appropriate training programs.
Day to Day Operations
- Oversees the day to day operations of theorganizational safety, including development of relevant policies and procedures with key stakeholders to guide practices
- Coaches and mentors staff and managers with regards to quality and patient safety emphasizing a just culture
- Seeks and manages external consulting arrangements including legal counsel and claims adjustors.
- Fosters an environment that values innovation.
- Ensures integration of the Occupational Health and Safety and Quality and Patient Safety initiatives.
- Acts as primary investigator for patient incident investigations where significant liability exists.
Secondary Responsibilities
- Facilitates the implementation of quality improvement activities and act as a resource to clinical program leaders in the development and maintenance of quality improvement processes.
- Leads/participates in process review, patient safety incident reviews and FMEAs related to quality/safety improvement in the clinical programs.
- Prepares and presents statistical reports of quality and patient safety activities to the clinical program leadership
- Liaises with Patient Relations on significant patient complaints that may have a risk component.
- Co-ordinates the delivery of appropriate education, mentoring and coaching in support of quality improvement and patient safety activities in the clinical programs.
Decision Making
- Interpretation of legislation and policies related to patient safety, quality and risk management for hospital-wide application. Set in place programs, policies and guidelines to address hospital-wide systemic gaps.
- Identification and resolutionof and Risk/Patient Safety risks that can be limited to specific programs (ie Lab, Medicine or DI) or have hospital-wide impact.
Make recommendations in relation to risk mitigation.
- Develops strategies and plans to improve patient safety culture across MGH. These initiatives will impact all units and departments.

Education
- BScN or related undergraduate health sciences degree required
- Masters degree in quality improvement or patient safety or health administration related field preferred
- Registered Nurse in good standing with the College of Nurses of Ontario preferred
- Certified Safety Specialist Certificate preferred
Experience
- Minimum of 3-5 years of experience in a quality improvement or patient safety initiatives and/or committees.
- Demonstrated ability to foster engagement culture of quality and patient safety.
- Excellent ability to establish trust and collaborativeworking relationships with stakeholders
- Advanced knowledge of relevant hospital and patient safety legislation.
- Excellent verbal and written communication skills, ensuring staff and colleagues are informed, involved and heard in an setting ofmutual respect.
- Demonstrated ability to manage concurrent complex projects and follow them through to completion, achieving desired organizational outcomes.
- Excellent analytical skills with a demonstrated ability to interpret and analyze data and identify trends and patterns.
- Demonstrated decision making, problem solving and priority setting skills, comfortable with ambiguity.
- Experience with RL Solutions Incident Management System.
- Good work and attendance record required
