- Establish therapeutic client relationships through effective communication
- Screen referrals and conduct individualized client assessments

- Provide health counseling and education on self-management
- Maintain clinical records and participate in quality improvement initiatives Requirements:
- Current BCCNM Registered Nurse registration
- One year of clinical experience with complex geriatric patients
- Valid BC Driver's license with personal vehicle access
- Robust ability to communicate and problem-solve effectively
