RPN – TRANSITION NURSE

Are you an experienced Registered Practical Nurse (RPN) looking for something different from traditional bedside nursing?

Wendat Community Programs is looking for a compassionate and experienced Transition Nurse to join our Psychogeriatric Program and support seniors throughout Simcoe-Muskoka.

WHAT YOU'LL DO

As a Transition Nurse, you'll help seniors successfully move from one care environment to another — whether that's a hospital, retirement home, private residence or Long-Term Care.

You'll be there before, during and after the transition to help set everyone up for success.

Your role will include

- Establishing trusting, therapeutic relationships with clients and families
- Assessing individual support needs
- Developing comprehensive, person-centred plans of care




- Identifying strategies to reduce the frequency and intensity of responsive/risk behaviours
- Supporting clients and families before and during transitions
- Collaborating with healthcare and community partners to prepare for a client's arrival
- Providing 3–6 weeks of hands-on support in the client's new environment
- Gradually transferring direct care through coaching and mentoring care providers
- Supporting the client as they adjust to their new living environment
- Working alongside staff in the new environment to ensure the client's unique needs are understood and met
- Supporting staff, families and care partners in implementing the strategies outlined in the care plan
- Maintaining accurate client and program documentation
- Contributing to program planning, development and evaluation
- Assisting with training new team members
- Participating in team meetings and committees

THIS IS MORE THAN A NURSING JOB

You'll have the opportunity to teach, mentor, advocate and lead — while still using your clinical nursing experience.

Rather than simply handing a client off to another care setting, you'll stay involved during the transition and help the receiving team understand who that person is, what they need and how to best support them.

Your expertise can help turn a challenging transition into a successful new beginning.

WE'RE LOOKING FOR AN RPN WHO...

- Is registered with the College of Nurses of Ontario




- Has 5 years of experience working with seniors
- Has experience with dementia, neurological conditions and/or responsive behaviours
- Has hands-on geriatric experience in LTC, hospital, community or retirement-home settings
- Understands behaviour-management strategies
- Has strong assessment and care-planning skills
- Is confident communicating with families, healthcare professionals and care teams
- Can build trusting relationships and work collaboratively across different sectors
- Demonstrates sound judgment when dealing with complex situations
- Is comfortable working independently and travelling throughout Simcoe-Muskoka
- Has a valid driver's licence and reliable vehicle
- French language proficiency is an asset

Salary

$54,800 – $66,400 per year

Starting salary based on nursing experience and qualifications.
