Registered Social Worker, Recreational Therapist, Practical NurseJob DescriptionPosition Overview: As a registered health professional (registered social worker, recreation therapist, or registered practical nurse) reporting to the Program Supervisor, you will provide tenants with short-term case management supports that help them navigate community and social services and build capacity to self-manage physical and mental health needs.Salary:$53,244-62,220 (Registered Social Worker/Recreation Therapist)Salary:$59,160-69,870 (Registered Practical Nurse)Hours of Work: Full time, 40 hours per week, 8 hours a day. Schedule is based on support needs of individuals in the program, including some evenings, weekends and public holidays. As the program operates 7 days a week, an on‑call rotation of carrying a work cell phone 24/7 for one week every five weeks is required.ResponsibilitiesCollaborate with interdisciplinary team staff to support shared decision‑making around case management and referrals.Provide on‑site, in‑home assessments and interventions to support long‑term stable housing through holistic supports.Develop and manage short‑term case plans using asset‑based and person‑centered frameworks.Support diverse individuals and communities with cultural safety, anti‑racist, anti‑oppressive and trauma‑informed practice.Advocate for tenants’ health needs, connect them to community and social service supports,



and educate tenants on system navigation and self‑advocacy.Compile up‑to‑date community and social service resources in the Greater Hamilton Area and surrounding regions, and facilitate information sessions and workshops.QualificationsDegree in Social Work, Diploma in Nursing (RPN), Recreation Therapy or a related discipline focused on mental health, addictions, or disability.Registered with a regulated college of professionals or willing to obtain registration.Experience working within a case‑management framework with individuals experiencing responsible mental illness, substance use, or other special needs.Knowledge of serious mental illness, addictions, concurrent disorders, dual diagnosis, mental health legislation.Familiarity with community and social services available in the Hamilton area and ability to work collaboratively with internal and external partners.Capability to develop and implement individualized care plans that support health self‑management and navigation of social systems.Leadership in coordinated care planning with internal and external supports.Proficiency in thorough, timely documentation and maintenance of tenant health records.Warm, sensitive, and caring approach with appropriate professional boundaries.Strong initiative, communication, and problem‑solving skills; ability to work independently,
