As a Program Navigator, you will advocate for residents in their journey toward independent living. You will assist in discharge planning, educate families, and develop vital psycho-educational support groups. This position requires building robust relationships with community partners and ensuring clients receive comprehensive support through their recovery process.
Key Responsibilities:
Advocate for and guide residents through recovery
Provide discharge planning and community resource referrals
Facilitate support groups and life skills training
Maintain accurate client records and progress assessments
Develop and maintain community partnerships
Requirements:
Diploma in community social services or equivalent
Experience with marginalized groups and substance use services
Valid first aid certification with CPR
Ability to administer Narcan and respond to overdoses
Solid communication skills and integrity
