WoodGreen Community Services is seeking a Part Time Client Support Worker for the Cummer Housing Program, on-site in Toronto. You will support seniors facing homelessness with housing, health, and social services, promoting independence and well-being.
You will build relationships, assist with care plans, crisis support, ADL/IADL assessments, and connect clients to community resources. Robust communication and de-escalation skills are essential, with 2+ years of relevant experience preferred.
