As part of the Centretown Community Health Centre, this position is essential for promoting the Community Diabetes Education Program. You'll work with CDEPO’s multidisciplinary team, guiding patients on insulin initiation and chronic disease management. Your experience in diabetes education and group facilitation will greatly contribute to our community outreach and support efforts.
Key Responsibilities:
• Deliver group and individual diabetes education sessions
• Support insulin initiation and medication management
• Collaborate with physicians and endocrinologists
• Conduct outreach activities across various locations
• Refer patients to appropriate health services
Requirements:
• Bachelor’s degree in Nursing or Nutrition Science
• Minimum two years’ experience in diabetes management
• Certification as a Diabetes Educator
• Solid interpersonal and communication skills
• CPR Certification and language skills are desirable
