As a Registered Nurse in our Home Health team, your expertise is crucial. Key Responsibilities:
• Establish therapeutic client relationships through effective communication
• Screen referrals and conduct individualized client assessments
• Develop shared patient care plans with the healthcare team




• Provide health counseling and education on self-management
• Maintain clinical records and participate in quality improvement initiatives

Requirements:
• Current BCCNM Registered Nurse registration
• One year of clinical experience with complex geriatric patients
• Valid BC Driver's license with personal vehicle access
• Solid ability to communicate and problem-solve effectively
• Commitment to promoting client-focused care
