Fraser Health Authority is seeking a Regular Part Time Chronic Disease Nurse to support the Diabetes Services team at Jim Pattison Outpatient Care & Surgery Centre in Surrey, BC. The nurse will plan and provide diabetes education, patient self-management coaching, and comprehensive chronic disease care within an interdisciplinary team.
The role requires an active BC RN license, two years of recent diabetes experience, and a valid CPR.
