As a key member of Bayshore's Integrated Care Solutions, you will engage with clients, families, and professionals to tailor support plans that address social, emotional, and psychological needs. This part-time position involves 2-3 days of daytime availability, approximating 15 visits weekly, ensuring a flexible schedule while providing essential services within the community.

Key Responsibilities:

- Coordinate care among clients, families, and service providers
- Manage a caseload efficiently, meeting priority timeframes
- Conduct assessments of clients' emotional and social challenges




- Develop and adjust psychosocial treatment plans with clients

Requirements:

- Active member of the Ontario College of Certified Social Workers
- Minimum of two years of community-based therapy experience
- Strong English communication skills; French is an asset
