Oak Valley Health is seeking a Dietitian to perform nutritional assessments and develop care plans for acute and subacute adult patients at Markham site and RCC site. You will implement evidence-based nutrition interventions and coordinate care across hospital to home transitions.
In this role you will lead within an interprofessional team, apply best practices in nutrition care, and support quality improvement initiatives while maintaining skilled standards and documentation in the EMR.
