Ontario Health at Home in Cornwall is seeking a Care Coordinator to assess patient care needs, develop care plans and coordinate home and community services.
You will work with patients, families and a multidisciplinary team across Eastern Champlain South, ensuring timely access to resources and transparent communication with physicians and providers.
Positions start with 35 hours per week, 8:30am to 4:30pm, and a starting salary of 44,480 to 47,517 CAD.
