As a critical member of the interdisciplinary team, you will provide social work services in an acute care hospital environment. Your responsibilities will include performing psychosocial assessments and short-term counseling for patients and families facing complex health challenges. You'll coordinate care plans in collaboration with healthcare professionals and address any service delivery gaps swiftly.
Key Responsibilities:
• Complete comprehensive psychosocial assessments
• Provide short-term support and counseling for grief and end-of-life issues




• Develop patient care plans alongside healthcare teams
• Cover shifts for other Social Workers as needed
• Liaise with community health professionals for resource connections
Requirements:
• Master's degree in Social Work
• Registration with Ontario College of Social Workers
• Recent hospital social work experience
• Robust communication and problem-solving skills
• Experience with electronic documentation, preferably Meditech
