This full time position requires a solid clinical background and the ability to facilitate communication within interdisciplinary teams. You will play a key role in patient assessments and developing therapeutic plans tailored to individual needs. Key Responsibilities:
Identify drug therapy problems with patient collaboration
Provide documentation and reporting on medication management




Educate patients and teams on drug-related issues
Support process improvements in patient safety
Conduct mentoring sessions for pharmacy students and residents Requirements:
Registration with College of Pharmacists of BC
Two years of patient care experience required
Valid Class 5 driver’s license necessary
Proven communication and organizational skills
