Sunnybrook is seeking a Registered Nurse with expertise in home dialysis to assess, plan, implement, advocate, and evaluate care for patients with end-stage kidney disease on home dialysis modalities. You will work as part of an inter‑qualified team to provide person‑centred care.
The role involves education for patients and families, home visits, data analysis, and ongoing support as a primary nurse or part of a team. A valid RN license in Ontario and BCLS are required.
