Key Responsibilities:
• Manage a patient roster of approximately 1400
• Provide comprehensive family medicine including preventive care




• Build long-term patient relationships and ensure continuity of care
• Engage in acute and chronic disease management
• Collaborate with a supportive practice team

Requirements:
• MD or equivalent medical degree
• Family Medicine training/certification or equivalent
• Eligible for CPSO registration in Ontario
• Strong commitment to patient-centred care
• Excellent communication skills
