Woolwich Community Health Centre is hiring a part time clinical nurse (RN) for 22.50 hours per week. This role covers day and evening shifts at multiple sites, focusing on essential patient care services.

As a Clinical Nurse at WCHC, you will provide evidence-based primary care and participate in a multidisciplinary team dedicated to enhancing patient outcomes. Your tasks will include health promotion, chronic disease management, and possibly prescribing medications as per your qualifications. Key Responsibilities:

• Conduct patient education and chronic disease management

• Prescribe medications as allowed by CNO standards

Requirements:
• Active RN registration with the College of Nurses of Ontario
• 2–3 years of nursing experience, especially in primary care
• Strong clinical assessment and patient education skills
• Interdisciplinary teamwork expertise
• Familiarity with electronic medical record systems
