This role focuses on delivering comprehensive clinical assessments, case management, and rehabilitation support. Ideal candidates will hold an active registration with the Ontario College of Social Workers and an undergraduate degree in Social Work. Proven clinical experience in the area of acquired brain injury is preferred, along with excellent communication and interpersonal skills.

Key Responsibilities:
• Conduct comprehensive clinical assessments for patient needs
• Develop individualized treatment plans for improved quality of life




• Engage in effective case management and rehabilitation support
• Collaborate with patients to foster meaningful community engagement
• Ensure compliance with safety policies and procedures

Requirements:
• Active registration with Ontario College of Social Workers
• Undergraduate degree in Social Work or related field
• Valid Ontario driver’s license and ability to travel
• Current clinical experience in acquired brain injury
• Excellent verbal and written communication skills
