Key Responsibilities:
• Conduct psychosocial and geriatric assessments
• Develop and implement coordinated care plans




• Provide case management and counseling services
• Facilitate crisis intervention for clients

Requirements:
• Masters of Social Work (M.S.W.) or relevant degree
• R.S.W. in good standing with OCSWSSW
• 3+ years in mental health and geriatrics
• Experience with older adults in care settings
