These positions require previous palliative care experience, and the nurses will work morning (8:00 - 15:00) or evening shifts (15:00 - 22:00). Delivering home nursing and end-of-life care, you will collaborate with an inter-professional team to ensure high-quality patient support.

Key Responsibilities:
• Assess and monitor patient care needs
• Evaluate and adjust care plans based on feedback
• Maintain documentation compliance with nursing standards




• Collaborate with healthcare teams for coordinated care
• Participate in team meetings and stand-by rotations

Requirements:
• Current registration with the College of Nurses of Ontario
• Experience in palliative or community nursing
• Robust communication and documentation skills
• Valid First Aid/CPR certification
• Ontario driver's license and reliable vehicle
