As a key member of Bayshore's Integrated Care Solutions, you will engage with clients, families, and professionals to tailor support plans that address social, emotional, and psychological needs. This part-time position involves 2-3 days of daytime availability, approximating 15 visits weekly, ensuring a versatile schedule while providing essential services within the community.

Key Responsibilities:
• Coordinate care among clients, families, and service providers
• Manage a caseload efficiently, meeting priority timeframes
• Conduct assessments of clients' emotional and social challenges




• Develop and adjust psychosocial treatment plans with clients
• Teach coping skills for stress, anger, and healthy grieving

Requirements:
• Active member of the Ontario College of Certified Social Workers
• Minimum of two years of community-based therapy experience
• Strong English communication skills; French is an asset
• Valid driver's license and access to a vehicle preferred
• Current CPR certification required
