Empower the Black communities as an Intensive Case Manager with CMHA Peel Dufferin. This on-site role focuses on holistic care and requires experience in mental health and addictions support.

This permanent full-time position involves providing individualized case management to clients with reliable mental health and addiction issues. You will facilitate access to primary care and community services, using a stepped-care approach. Additionally, working within a multidisciplinary team, you'll develop care plans and conduct regular assessments.

Key Responsibilities:
• Conduct needs assessments and monitor care plans
• Provide task-based interventions related to social determinants of health
• Connect clients to crisis services when needed




• Collaborate with the multidisciplinary team for care planning
• Facilitate client attachment to primary care providers

Requirements:
• Post-secondary degree in relevant discipline
• Minimum 2 years of experience in mental health
• Expertise in community resources and support
• Strong cultural sensitivity working with BAC communities
• Valid Canadian driver's license and police check
