Join CMHA-YRSS as a Registered Practical Nurse on the Assertive Community Transition Team. Your expertise in mental health nursing will support recovery and well-being for clients in Vaughan.

This full time, one-year contract role requires a commitment to providing high-quality care and support for individuals with mental illness. As an RPN, you will collaborate with a dedicated team, conducting assessments and developing treatment plans for clients aged 16 and older. Your role will focus on medication management and enhancing client empowerment through a recovery-based approach.

Key Responsibilities:
• Conduct mental and physical health assessments
• Manage and administer medications in collaboration with the psychiatrist




• Develop individualized treatment plans for clients
• Coordinate care and connect clients to community resources
• Foster collaborative partnerships with clients and families

Requirements:
• Licensed Registered Practical Nurse (Dipl. N.)
• Four+ years of mental health experience preferred
• Strong health assessment and clinical skills
• Knowledge of Ontario mental health legislation
• Valid Canadian driver's license with insurance
